- 02 bản sao tác phẩm đăng ký quyền tác giả hoặc 02 bản sao bản định hình đăng ký quyền liên quan.
- Thông tin người nộp đơn.
- Văn bản đồng ý của các tác giả (nếu có đồng tác giả)
Sau 15 – 20 ngày làm việc kể từ ngày nhận được hồ sơ hợp lệ Quý khách hàng sẽ nhận được Giấy chứng nhận đăng ký bản quyền thiết kế áo đồng phục..
